Our care managers are available on an on-going basis to assist our clients with custom tailored services designed to meet their respective care and lifestyle needs. Many of our clients are elderlaw attorneys who are court appointed Guardians for individuals in nursing homes and assisted living facilities. They have asked us to assign a care manager to monitor the care and services of their client by making weekly visits, attending care planning conferences, advocating on behalf of their client with facility staff, accompanying clients to medical and dental appointments, shopping and even social occasions. These are but a few examples of the many diverse services provided by our care management staff.

We provide similar services for clients who are long-distance caregivers concerned about the safety and well-being of their parents or loved ones living at home or in a long term care facility. Visits and monitoring services are provided on a regular schedule and if necessary on an urgent basis as well. Our care managers maintain close reporting relationships with all clients to ensure that factual and timely information is communicated regularly. Monthly reports are prepared and delivered to all clients. Our care managers will supervise and coordinate various services provided by other agencies and vendors at the specific request of our clients.

Our care managers are available each day, around the clock, to respond to each of our client’s concerns. In conjunction with our care management services, we have developed certain exclusive referral relationships with several home care agencies in the greater Boston area that have met and exceeded our high standards for quality of care. By working in collaboration with these premier agencies, we are able to respond to an urgent need for service quickly and with fewer problems.
